Before you get started...

This screening is not a formal diagnosis but serves as an initial assessment to determine if further evaluation by educational or healthcare professionals may be beneficial.

The screening is divided into two distinct sections:

Section 1: Parent Questionnaire

This is to be completed by the child's parent/guardian only. Focuses on observations of your child’s learning behaviours and developmental history. This section will take approximately 5 minutes to complete. 

 

For the best results, please be honest in your responses: The most helpful results come from accurate observations​

Section 2: Child Assessment Activities

You are required to facilitate the assessment with your child. This section focuses on phonological awareness, reading,  comprehension, spelling and writing abilities.

Requires your child’s active participation with your guidance. This section takes approximately 10 minutes to complete. It's important that your child is in a comfortable environment and relaxed state.  You can explain the purpose of the assessment in simple terms and encourage your child to do their best, but reassure them that it’s okay if they find some tasks difficult.
